The middle initial E. was supplied from the nameplate of the organ in North Baptist Church, New Bedford, Massachusetts (OHS Database ID 8529, with photograph of the nameplate): Chas. E. Chadwick Reading, Mass. 1901 Barbara Owen states that Chadwick moved to Reading in 1902, but the nameplate appears to show that he moved there not later than 1901.
